Jeffrey A. Collinsworth

Jeffrey A. Collinsworth, 46, of Middletown, died Wednesday, September 30, 2009, at Mercy Hospital Fairfield. He was born in Middletown on November 24, 1962 to parents Clarence and Mildred (Gayhart) Collinsworth.

Mr. Collinsworth is survived by his children, Jeff (Jeanna) Collinsworth Jr., Michael (Amberly) Collinsworth, Dustin Collinsworth, Jeremy (Stephanie) Caudill and Adam Collinsworth; brother, Tony Collinsworth; sister, Tina (David) Pryor; niece, Jessica Gingerich; nephews, Tony Collinsworth Jr. and Cris Collinsworth; and 7 grandchildren.

He was preceded in death by his parents.

Memorial service will be held Sunday, October 4, 2009 at 4:00 pm at WILSON-SCHRAMM-SPAULDING FUNERAL HOME, 3805 Roosevelt Blvd., Middletown. A gathering of family and friends will be prior to the service from 2:00 - 4:00 PM.
