James "Jd" Sivak

James David Sivak, 59, of Lebanon, died on Saturday, December 31, 2022 at Atrium Medical Center. He was born in Middletown on December 3, 1963. JD was a store manager for PetSmart.

JD loved his family and will be greatly missed by his wife, Brenda (Gilbert) Sivak; son, Ryan (Barbara) Sivak; daughters, Amber (Doug) Sivak & Sierra Sivak; step-daughters, Kristen (Jobey) Parks & Breanne Gilbert; his mother, Joy Elaine (Coldiron) Sivak; brothers, Mike Sivak, Jr. & Steve (Jennifer) Sivak; sisters, Theresa (Kenny) Bowman, Paulette (Larry) Reed & Elaine (Joe) Kinnard; and two grandchildren, Logan Ryan Burns & Suede Tudor.

He was preceded in death by his father, Francis "Mike" Sivak.

Funeral Service will be Saturday, January 7, 2023 at 12:00 noon at Crosspointe Church of Christ, 5630 State Route 122, Franklin, with Reverend Scott Johnson officiating. Visitation will be one hour prior to the service from 11:00 am - 12:00 noon at the church. Interment will be at Woodside Cemetery and Arboretum.
